Instructions for Filing Annual Reports

CH-8: NAPNAP Chapters' Authorization for NAPNAP to File with the IRS on the Chapter's Behalf

A letter of authorization for inclusion in the group tax return. If this form is not submitted on time, the chapter will not be considered covered by the group exemption tax return filed and the chapter may be required (by the IRS) to pay income tax for income earned that year. Please note: this form must be signed by the outgoing officers and the original signatures must be forwarded to the national office. Electronic filers: if approved by your executive board only one signature will be required from your officers. This form is due in the National Office by August 1.

CH-8A: Chapters Member List Form

A list of chapter members for the membership year just completed (March 1, 2007 - February 28, 2008). This is very important as it will be used in determining the chapter fee to be invoiced, as well as the winners of the Chapter Conference Scholarships. This form is due in the National Office by August 1.

CH-9: Annual Chapter Financial Statement Form

An annual financial statement for inclusion in the group tax return. If these forms are not submitted on time, the chapter will not be considered covered by the group exemption tax return filed and the chapter may be required (by the IRS) to pay income tax for income earned that year. The original signatures must be forwarded to the national office. Electronic filers: if approved by your executive board only one signature will be required from your officers. This form is due in the National Office by August 1.

CH-10: NAPNAP Chapter Officers List Form

This report lists all the officers and committee members who will serve for the fiscal year beginning July 1. In order for the Executive Board, prospective members and the National Office to keep the chapter informed, this form is due in the National Office by June 1.

CH-12: NAPNAP Chapter Activites Form

A list of the activities of your chapter during the past year. This will be used to determine the winners of the Annual Large and Small Chapter Awards, so please be sure to make your reports generic. In Section II, Awards Criteria, do not use an individual's name, your chapter's name, or make any reference to a specific geographical area. List three items you wish to be considered for the Annual Large and Small Chapter Awards. This form is due in the National Office by August 1.

PLEASE NOTE:

The CH-10 is due in the National Office on June 1. All other reports must be received by the National Office by August 1. Annual Chapter Fees are due by September 1. Failure to have the reports in the National Office by the date due will result in your Chapter being ineligible for Chapter Awards and possible action by the NAPNAP Executive Board.
